Adding a answer choice to free text answers

Question

In my survey I am asking a question and would like my subjects to reply in a text field. Additionally I would also offer to not answer at all and for this I wanted to create a selection with only one possible selection. How can I implement that?

you could just combine an text field with a simple single choice question underneath.

Please create a question for open ended answers (text input) and select the option to provide a residual option, either in the question or in the item.
