Looks like you manually set a width that is much too small for the options in the "Detail Settings", try 160 pixels (or something like that), please.
the second-to-last page has a prompt which is not supposed be there.
It seems that the page shows exactly what you told it to show. What do you mean by "prompt"? If it's the "next" button, add
option('nextbutton', false);
