DPA form error

Question 1

Since I want to collect participants' email addresses, I'm required to sign DPA. When I fill the form and request "prepare contract for signing" I receive the following error:

Warning: Please enter the full postal address of the responsible
party.

Does the address needs to be entered in a specific format? I have entered the full address of my university in addition to zip code.

Question 2

Please write each part of the address (street + town) in a separate line.

If both is in one line, SoSci Survey will think that you have entered only the street.
