Form to autenticate me for the use of media files won't send.

I have tried multiple times to send a form to authenticate my account for the use of audio files, but every time I try to send the form a message pops up that states: Error: The server was unable to validate that you actively triggered the action. Please confirm that you are working on the right project and double check the data. If everything is fine, please submit, again

How do I fix this and send the form?
