Questions not showing up

Why are only three of my questions showing up when I have 9 categories? Only one category is showing up on my survey.

Please make sure that

(1) you have placed all the questions in Compose Questionnaire and

(2) that there is only one questionnaire, and you're not running another one that has only three questions.

If neither is the solution, please go more into detail: On which page do you miss which questions and what does the debug information tell you?
