Did you maybe import the "Socio-demgraphics" package? And did you also import the sample questionnaire with the package?
In that case, you have more than one questionnaire in your project, and obviously, the wrong one is the "default".
To change the default (or delete superfluous questionnaires), please go to Compose Questionnaire -> Manage Questionnaires.
(although at the moment, it looks like there is no demo/default questionnaire behind the link).
