It seems that you're using some PHP code on the first page of your questionnaire, that is not working.
Please check if you have more than one questionnaire in your project. If you have, then please ensure that the correct one is chosen as default (last tab in "compose questionnaire"). If none was chosen as default, SoSci Survey may use a different one than you wanted it to do:
SoSci Survey used to choose the questionnaire (if none was explicitly selected) by alphabetical order. While this is still the case, an update changes this behavior from case-sensitive to case-insensitive. That means: When "Beta" was selected before "alpha" (because "B" comes before "a"), now "alpha" is selected before "Beta".
Whenever you have more than one questionnaire in your project, you should always set a default.
