Collecting personal information separately

I have a problem in creating a question to collect information such as first name, second name and student ID separately. as for the function of email address.

Is there any solution for this?

When using the question type that collected information separately, you can choose whether to accept only email addresses, or an input. If you allow for any input, it's useful to give the input some height (for example 70 pixels), and use a label such as

First name:<br>Second name:<br>Student ID:
