I added a reference to the links with the questionnaire (such as ?=U2 for example) but the results did not show any reference number which makes it hard to attribute the answers to the case.

The r (reference) is important: The correct syntax would be ?r=U2, and if this is not the first parameter, it would be &r=U2: URL to the Questionnaire
